Beyond Hospital Walls: SeniorConnect, Remote Patient Monitoring For Your Loved Ones
Digital tools are reshaping the way patients receive, access and experience healthcare, driving hospitals to embrace the rapid advancement of digital technologies. From electronic health records to artificial intelligence-powered diagnostics, the availability of cutting-edge technology serves the best interests of patients. This is only the beginning to a much digitalised healthcare system.
Subang Jaya Medical Centre( SJMC) is changing the way care is provided through SeniorConnect, a remote patient monitoring service tailored to the unique needs of senior patients. Through this service, healthcare professionals can accurately record and closely monitor individuals’ vital signs daily and deploy immediate help if necessary. The advanced remote monitoring system ensures that seniors receive the necessary care and attention, promoting their well-being and peace of mind to their loved ones.
While traditional healthcare model focuses on treating diseases and is seen as the primary setting for healthcare delivery, SJMC is shifting this train of thought from patients to people, from healthcare to health, from hospital to home – breaking this cycle by empowering individuals to take charge of their own health, beyond hospital walls and extending healthcare into homes.“ Imagine having hospital-level medical services with you at every corner of your home, no matter where you are, allowing you to receive treatment and care comfortably and conveniently. In concept, SeniorConnect can be a hospital that transcends all physical boundaries and borders,” said Bryan Lin, SJMC Chief Executive Officer. In today’ s fast-paced world, it is often challenging to keep a

close eye on your loved ones, especially those who are elderly or may have chronic health conditions. Being in the hospital can be hard on some seniors – not being near to their loved ones and feeling lonely in a sterile environment. Today more than ever, patients have a number of choices to make about the type of medical care they receive.
Since the introduction of SeniorConnect, SJMC Consultant Nephrologist Dr Kenneth Lai has been using remote patient monitoring system to monitor the health conditions of patients with chronic kidney disease during the diagnosis and treatment process.
One of his patients struggled to maintain her blood pressure, despite trying various medications. Eventually, she gave up on medication, causing her kidney failure to worsen. Since using SeniorConnect, her blood pressure has been effectively managed through regular monitoring and medication adjustments by the doctor. This has also led to improved kidney function. Her confidence in managing her condition has enabled her to successfully discontinue SeniorConnect after one month.
“ With remote patient monitoring service now available, patients can conveniently monitor their health conditions regularly at home. Patients only need to return to the hospital regularly for physical therapy, rehabilitation, or follow-up visits, and use remote patient monitoring system at home to continue monitoring their health conditions,” Dr Lai said.
SeniorConnect also allows families and caregivers to stay connected and informed about their loved ones’ health status from anywhere, anytime. Janice Lee shares that she and her sister are sole caregivers to their 92-year-old grandmother and being working adults, it was often worrying to leave their grandmother alone with a helper in the event of a medical emergency.
“ Once, the SeniorConnect nurse observed an elevated blood pressure and alerted me. My grandmother had no complaints of discomfort that day however her legs were swelling and the nurse advised me to bring her to see a doctor. She was admitted for monitoring and was discharged after two days. I am glad the nurse altered me before her condition took a turn for the worse. SeniorConnect gave my family a peace of mind, knowing that a trained medical personnel is watching over my loved one,” Janice said.
SJMC embraces a transformative approach to patient care that leverages technology to improve outcomes. Remote patient monitoring is paving the way for a more digitalised healthcare system by empowering individuals to take an active role in their health management while transcending all physical boundaries of a traditional hospital.
As an award-winning multi-disciplinary hospital, SJMC has served the Malaysian community for 39 years with comprehensive and complex care across all medical specialities.
